Job Description:
We are looking for a responsible, supportive, and compassionate Community Support Professional to join our growing team.
The ideal job candidate will be highly dependable and have a passion for helping those with physical and/or mental disabilities live a more fulfilled and happy life.
The Community Support Professional will also be responsible for helping them to improve and meet individual specific goals, while insuring that all internal and external agency guidelines are met by providing supports in their community.
Duties:
Assist individuals with disabilities to engage and progress towards independence in their community through the Integrated Community (IC) program and Teleservice supports.
Create and implement a monthly calendar to support individuals authorized for IC and Teleservices including but not limited to volunteerism, community memberships, civic opportunities, personal relationships, independent living skills, employment skills, and other avenues as identified by individual interests and desires.
Insure client supervision, health, safety, development and quality of care according to client's Individualized Service Plan (ISP) or other care plan.
Encourage and foster individuals' preferred goals and interests.
Assist with and work to strengthen the individuals' many daily living needs including but not limited to:
nutrition, hygiene, exercise, communication and socialization skills.
Develop meaningful relationships and connections with your clients to make them feel cared for and included in the community.
Transport clients to other volunteer opportunities, outings and other events Connect client with community resources and natural support system through activities, outings and other events.
Qualifications:
- High school diploma or equivalent - Valid Driver's License - CPR/ First Aid Certification - Familiarity with medication administration procedures preferred - Insurability on company auto insurance -Ability to pass pre-employment ODOT physical and drug testing
